Dylan Memorial Shirt Order Form – Instructions
 
We are deeply sorry to hear about the passing of Dylan, and we extend our heartfelt condolences during this difficult time.
 
This form is being used to place orders for the Dylan Memorial Shirt. Please review the following important information before submitting your order:
 
One order per person: Each individual must submit their own order using this form. Please feel free to share this link with anyone who may wish to order a shirt.
Shipping: All orders being sent to the same address will be packaged and shipped together.
Pricing: Each shirt is $35, which includes free shipping and handling. A portion of the proceeds will be donated to Dylan’s family.
Design Approval: Once the shirt design is finalized and approved, any requested updates will be made. At that time, orders can be placed using this link.
Accuracy: Please ensure all information entered, including size and shipping address, is correct prior to submission.
